The Faith Community Nurse (FCN) at House of Hope and First Lutheran Church of Crystal addresses the
needs of the whole person (body, mind and spirit) across their lifespan within the context of our faith
communities. They work collaboratively with caregivers, ministry and health care professionals. They
demonstrate compassion and understanding while working with all age groups, especially older adults. They
are comfortable making introductory calls and visits. They feel confident talking about faith and end of life
decisions.
Preferred Qualifications
- Current license as an RN in Minnesota. BSN preferred. Retired nurses welcome to apply.
- Completion or willingness to complete FCN program
- Strong Christian faith and calling into this field of ministry
- Valid Driver’s License
- Basic computer skills

- Able to utilize needs assessment and evaluation tools
- Holding confidentiality alongside congregational pastors
- Self-motivated and self-directed life-long learner
Job Details - Hours and pay: The position averages 10-12 hours per week.
- Pay starts at $30/hr depending on qualifications.
- Hours are flexible and regularly discussed with pastoral leadership
- Visit worship at each congregation one Sunday per month.
Responsibilities
Visitation - Visit members in their homes or care facility in person and on the phone
- Accompany members with holistic health concerns over time through regular contact
- Develop long term professional relationships with members through visits
- Develop priorities and visitation schedules by coordinating with each pastor in each congregation
Personal Health - Provides individual health counseling related to health maintenance, disease prevention and illness patterns
- Provides support to care givers of clients
- Clear and well communicated professional boundaries
Collaborative Health - Organize ongoing caregiver support and respite care
- Network with other parish nurses and ministry professionals to develop community contacts and referrals
- Provides referrals to health care services and resources within the community
- Encourages clients to avail themselves of services that enhance their overall wellbeing
Health Advocate - Assists and supports clients and client families in making decisions regarding their health, medical services
and care facilities - Recognizes the need for new health issue interventions and guides the process toward resolution
- Provides guidance for seniors and care givers during transitions in care
