Ecumen is a nonprofit organization dedicated to providing innovative housing, health care and community-based services that support older adults and strengthen communities. Mission-driven since its founding in 1862, Ecumen’s legacy of service reflects a commitment to honoring those it serves. Ecumen is based in Shoreview, Minn., and provides housing options such as independent living, assisted living, memory care, short-term rehabilitation and long-term care. Lyngblomsten's mission is influenced by Christ, Lyngblomsten provides a ministry of compassionate care and innovative services to older adults in order to preserve and enhance their quality of life.

Founded in 1906, Lyngblomsten is a Christian nonprofit organization providing healthcare, housing, and community-based services to older adults. We offer a continuum of living options at our campuses in the Como Park neighborhood of St. Paul and in Lino Lakes.

  • Our Como Park campus includes HUD-subsidized independent living at the Lyngblomsten Apartments, market-rate assisted living at The Heritage at Lyngblomsten, and transitional rehab and skilled-nursing care at the Lyngblomsten Care Center. ·
  • Our Lino Lakes campus offers independent living, assisted living, memory care, and overnight respite care.
  • Lyngblomsten Community Services provides support and resources to family caregivers of older adults who are living at home, specializing in serving those with memory loss and those who care for them. Offerings for family caregivers include local resource recommendations with options tailored to a caregiver’s unique situation, one-on-one and group education opportunities, and memory loss caregiver support groups. In addition, Community Services offers The Gathering, an award-winning group respite program for adults with early- to mid-stage memory loss that provides a four-hour break to their family caregivers.

Faith Community Nurse Network is honored to be one of approximately a dozen health organizations which the University of Minnesota - School of Nursing has a Collaboratory Agreement with. We have spoken in the classroom about Faith Community Nursing, hosted students as part of their community health clinical rotations, and worked with Doctorate of Nursing students on quality improvement projects.

Funding Partners

Metropolitan Area Agency on Aging distributes Older American Act funding to providers in the Twin Cities Metro Area. FCNN receives Title III D funding to support the Evidence-Based Health Promotion Programs we offer to older adults and their caregivers.

Minnesota Board on Aging currently funds the work of FCNN through two grant programs.

- Age-Friendly Minnesota: Our Age-Friendly MN grant supports our efforts in creating awareness about faith community nursing, community wellness programming, and assisting marginalized communities in expanding their wellness programs.

- Dementia: Our Dementia Grant support our continued efforts in creating a dementia-friendly community, promoting education and awareness to professionals as well as the community in general, providing cognitive assessments by way of our whole-person wellness screenings, and caregiver empowerment workshops.
