These resources will not work for each individual and is not all inclusive - there are many high-quality community resources in the Twin Cities.

We are blessed to work alongside many organizations and institutions within the Twin Cities Metropolitan Area - These organizations are a tremendous assets and resources to our community.

County Agencies in the Twin Cities Metro Area

The County is a good resource for helping individuals get connected to services and assistance like Elderly Waiver, Emergency Assistance, Food Assistance, etc.

Networks and Referral Organizations

  • While most resources that MN Aging Pathways connects people to are for older adults (caregivers and medicare), all ages are welcome to use service.

  • LAHN (Formerly Block Nurse Programs) transforms the landscape of healthy aging across Minnesota by empowering our community-based partners with resources, advocacy and collaboration. Members include SE Seniors, Northeast Seniors, Eastside Seniors,Hamline Midway Seniors, etc. Common services include offering companionship and social events, transportation, food services, household upkeep, and many more.
