Description: This session provides current information focused on nursing interventions to promote positive health behaviors to improve diabetes health outcomes. The discussion includes the spiritual aspects of living with diabetes.
FCN Scope and Standards of Care: The session addresses Standard:5B: Health Teaching and Health Promotion
Learning Outcomes: As a result of attending, participants will be able to:
1. Describe the components of diabetes management used for facilitating positive health behaviors to improve diabetes health outcomes. (taking medication, monitoring, eating, activity, coping and problem solving.)
2. Identify the most common current treatments and technologies used in diabetes care.
3. Discuss how faith community nurses can assess needs and help health care consumers improve their diabetes management.
4. Identify spiritual needs and spiritual care for health care consumers with diabetes and their loved ones.

This session is approved for three (3) Continuing Education Hours for RNs as required by the Minnesota Board of Nursing. Other health professionals are responsible for determining if this approval meets their licensure requirements.
SPEAKER BIOGRAPHY:
Faith Pollock, APRN, ACNS-BC, BC-ADM, CDCES
Faith has been a certified diabetes educator since 1996; a board certified adult clinical nurse specialist (CNS) with a focus on advanced diabetes management since 2008 and an APRN provider with board certification in advanced diabetes management as part of the Abbott Northwestern Hospital Endocrinology Team in Minneapolis since 2015. In addition to being a provider, her role is to improve and provide safe glucose management of inpatients by collaborating with nurses and providers in medication management, diabetes treatments and complex care coordination for transition of care.
