What we collect:
The FCNN collects the following personal information from our donors: donor name, acknowledgement name, amount donated, address, telephone number, and email address. This information is kept securely on file for IRS purposes.
How we use information:
Donor information is used for internal purposes only. The FCNN does not sell or lease donor information. Donor information will only be used by the FCNN’s staff to:
- Distribute receipts for donations
- Thank donors for their contribution
- Inform donors about current and upcoming activities of the FCNN including additional opportunities to support the FCNN
- Track and analyze donor data
- Comply with current rules and regulations governing 501(c)(3) financial reporting
- Comply with any reporting requirements related to specific grants or contributions
Where appropriate, and with the knowledge and consent of the donor, FCNN may publicly recognize the receipt and purpose of an individual gift. However, unless desired by the donor, FCNN will not disclose the amount of such gifts, except where required by law. Data on donors and donation amounts may be aggregated and stripped of identifying information for reporting purposes (including grant applications and government filings) without the explicit consent of the donors. We do compile an Annual Report every year and list donors in giving categories.
Safeguards:
- Office policies are in place to safeguard donor information, including the following:
- Physical measures, including locked filing cabinets and restricted access to offices.
- Organizational measures, such as security clearances and limiting access on a “need to know” basis.
- Technological measures, such as the use of passwords and encryption.
- A records retention policy.
The Donor Bill of Rights:
- To be informed of the Faith Community Nurse Network’s mission, of the way the Network intends to use donated resources, and of its capacity to use donations effectively for their intended purposes.
- To be informed of the identity of those serving on the Faith Community Nurse Network’s Board of Directors and to expect the Board to exercise prudent judgment in its stewardship responsibilities.
- To have access to the Network’s most recent financial statements.
- To be assured their gifts will be used by the Network for the purposes for which they were given.
- To receive appropriate acknowledgement and recognition of all contributions to the Faith Community Nurse Network.
- To be assured that information about their donations is handled with respect and with confidentiality to the extent provided by law.
- To be informed whether those seeking donations are volunteers, employees of the organization or hired solicitors.
- To feel free to ask questions when making a donation and to receive prompt, truthful and forthright answers.
