Ministry Replication Module
Developed by the Normandale Ministry for Healing & Wholeness as a part
of the Supporting Seniors Across Systems project, the module helps
faith communities develop care team ministries.
Community Based
Nurses & Faith Based Providers
Two fliers in one file. The first flier explains the different
community based nurses including faith based (parish), homecare,
public health and living at home / block nurses. The second flier
explains various faith based programs available for seniors including
faith based (parish) nurses, health / care team ministries,
Befrienders, Faith in Action and Stephen's Ministries.
Minnesota Advanced Health Care Directive (Off
Site)
The
materials in the packet contain tools to help consumers work through
the advanced care planning process. A suggested form and instructions
for completing the Minnesota Health Care Directive are included.
Parish Nurse
Effectiveness Study
The Metropolitan Area Agency on Aging received funds through the
Community Service/Services Development grant program from the Minnesota
Department of Human Services to analyze the interventions of parish
nurses on older persons and discern their impact on health care costs.
Using the DIARY methodology 75 parish nurses documented activities that
often resulted in preventing the need for expensive acute or long-term
health care while promoting independence and emotional well-being.
Planning Ahead
Includes information about legal planning tools, durable power of
attorney, trusts, guardianships and conservatorships, health care
directives, medical assistance, funeral planning and wills. |
